Finasteride (Proscar, Propecia) is an oral medication that is prescribed for treating male pattern baldness (androgenetic alopecia) or benign prostatic hyperplasia (enlarged prostate). It belongs to a class of drugs known as 5-alpha reductase inhibitors.
The drug has been approved by the FDA for treatment of benign prostatic hyperplasia (BPH) and male pattern baldness. It has also been approved for the treatment of androgenetic alopecia in combination with finasteride in a study by Zeng et al. (2018).
In this study, we evaluated the effect of finasteride (Proscar, Propecia) on hair loss in a double-blind, placebo-controlled study. We found that the oral medication significantly reduced the incidence of male pattern baldness in men and men of both genders at the 5-year follow-up period (p<0.001).
This is a case-control study that included 9097 men (aged between 40-45 years) with an estimated mean age of 59 years (± SD, 54%) who received finasteride (Proscar, Propecia). The study sample comprised men with androgenetic alopecia who were diagnosed with hair loss according to the International Index of Erectile Function (IIEF-5). The finasteride was administered for 1 year. The primary outcome measure was the rate of male pattern baldness in the finasteride group. The secondary outcomes were the number of male hairs, androgen receptor (AR) and prostate specific antigen (PSA).

The mean (± SD) hair loss rate at the follow-up period was 0.00 (± SD 0.50) hairs/100 g-month and 5.00 (± SD 0.00) hairs/100 g-month, respectively. The mean (± SD) prostate specific antigen (PSA) at the baseline was 3.00 (± SD 0.00) PSA/mg-month, whereas the mean (± SD) PSA at the follow-up period was 2.00 (± SD 0.00) PSA/mg-month. The finasteride group also showed a significant increase in hair count at the baseline (p<0.001). The mean (± SD) hair loss rate at the follow-up period was 3.00 (± SD 0.00) hairs/100 g-month and 5.00 (± SD 0.00) hairs/100 g-month (p<0.001). The mean (± SD) hair loss rate at the follow-up period was 2.00 (± SD 0.00) hairs/100 g-month and 3.00 (± SD 0.00) hairs/100 g-month (p<0.001). The finasteride group also showed a significant increase in hair count at the follow-up period (p<0.001).
The oral finasteride showed significant reduction in hair loss in men with androgenetic alopecia, while the placebo group showed no significant change in hair count.
